While there is some consensus regarding evidence-based interventions to successfully facilitate transitions of care and improve patient outcomes, these interventions are not always reliably implemented. This series will feature three transitions of care subtopics that will describe various types of transitions of care with respective best practices. It will also provide learner with a comprehensive overview of best practices and a reliable strategy for implementation for patient transition in, out, and within the hospital.
