Rapid Clinical Updates: Antibiotic Stewardship in Pneumonia

This session will provide an overview of the most up-to-date guidelines and antimicrobial stewardship principles as they apply to diagnosis and management of community-acquired, healthcare-associated, and ventilator-associated pneumonia. Participants will learn how to improve diagnostic accuracy, optimize empiric therapy, de-escalate and shorten antibiotic duration as appropriate, and understand the literature behind current guidelines.

Mounting evidence shows that the American healthcare system costs too much for the results it achieves in terms of patient outcomes and access. Evaluation and Management of Cirrhosis in the Perioperative Setting

Patients with cirrhosis who undergo surgery are at increased risk for complications. Perioperative morbidity and mortality vary depending on the severity of cirrhosis, type of surgery, and whether it is elective or emergent. Identification of cirrhosis and accurate assessment of severity can be challenging but is necessary for case selection, informed consent, and anticipation of potential complications. Inaccurate risk assessment can sometimes lead to inappropriate exclusion of patients with cirrhosis from receiving useful surgery, as well. For instance, reliance on the Child-Pugh score alone likely significantly over-estimates perioperative mortality in patients with cirrhosis undergoing most procedures. This activity identifies updated tools for perioperative risk prediction, outlines the most important elements of preoperative evaluation and optimization, and assists in formulation of pre- and postoperative care plans for patients with cirrhosis. Though our main focus is on cirrhosis, we will also touch on some important topics related to other acute and chronic liver disease.

Rapid Clinical Updates: Striving for Oral Stepdown: Novel Agents and Strategies for Avoiding OPAT

Outpatient Parenteral Antibiotic Therapy (OPAT) has served a key role in the treatment of severe and difficult to treat infections such osteomyelitis and bacteremia. However, use of prolonged antibiotic therapy can be associated with line related complications, prolonged hospitalization, and logistical challenges for follow up. This session seeks to review the latest data on long acting infusion based antibiotics and oral therapies with high oral bioavailability that can safely serve as alternatives to intravenous treatment.

Rapid Clinical Updates: Delirium and Inpatient Psychotropics

This session will discuss the burden of delirium among hospitalized patients, current preventive and therapeutic approaches to delirium, and the long-term impact of delirium. We will also discuss strategies for prevention of delirium in the hospital setting and how to recognize delirium in hospitalized patients, as well as pearls and pitfalls of pharmacologic and non-pharmacologic strategies to manage delirium and behavioral and psychological symptoms of dementia (BPSD). At the end of the presentation, the audience will be able to identify delirium, assess its severity, and appreciate the long-term adverse consequences of delirium.
