HQPS Transitions of Care Series

There is significant variability in the facilitation of transitions of care both in the hospital and out of the hospital to home. While there is some consensus regarding evidence-based interventions to successfully facilitate transitions of care and improve patient outcomes, these interventions are not always reliably implemented. This series will feature three transitions of care subtopics that will describe various types of transitions of care with respective best practices. It will also provide learner with a comprehensive overview of best practices and a reliable strategy for implementation for patient transition in, out, and within the hospital. The learner will gain the knowledge and tools to effect change in their institution and individual practice.

Choosing Wisely: Implementing Quality Improvement Projects addressing Choosing Wisely Topics

Mounting evidence shows that the American healthcare system costs too much for the results it achieves in terms of patient outcomes and access. Many different parts of this system need attention, and it can be difficult to know where to begin. However, the inpatient / hospital environment is a “high intensity” healthcare delivery setting, and the “Choosing Wisely” initiative of the American Board of Internal Medicine Foundation (ABIMF) offers a list of low value healthcare practices which make great targets for improvement efforts.

SHM's Clinical Quick Talks

In light of the busy clinical services and demands on hospitalists’ schedules, it is often difficult to carve out time to prepare short lectures for residents and students. To meet this need, SHM's Education Committee created a library of quick talks designed to be succinct (given in less than 10 minutes) for use during teaching rounds, a brief sit-down session, or topic review. Access to this library is free for active SHM members, and nonmembers can subscribe (3 years for $199).

DEA Requirement - OUD and SUD Education

The US Drug Enforcement Administration (DEA) recently announced updated rules for all registered clinicians. Starting June 27, 2023, clinicians applying or renewing their DEA registration will need to make a one-time attestation that they have attended at least eight hours of approved training. This will be due at the time of renewal, which will vary by clinician. As an accredited CME provider, the Society of Hospital Medicine has compiled a list of sessions that meet this requirement.
