HQPS Transitions of Care Series

There is significant variability in the facilitation of transitions of care both in the hospital and out of the hospital to home. While there is some consensus regarding evidence-based interventions to successfully facilitate transitions of care and improve patient outcomes, these interventions are not always reliably implemented. This series will feature three transitions of care subtopics that will describe various types of transitions of care with respective best practices. It will also provide learner with a comprehensive overview of best practices and a reliable strategy for implementation for patient transition in, out, and within the hospital. The learner will gain the knowledge and tools to effect change in their institution and individual practice.

Reducing Unnecessary Telemetry Monitoring: An SHM Choosing Wisely Quality Improvement Implementation Module

Mounting evidence shows that the American healthcare system costs too much for the results it achieves in terms of patient outcomes and access. Many different parts of this system need attention, and it can be difficult to know where to begin. However, the inpatient / hospital environment is a “high intensity” healthcare delivery setting, and the “Choosing Wisely” initiative of the American Board of Internal Medicine Foundation (ABIMF) offers a list of low value healthcare practices which make great targets for improvement efforts. In this activity, we will tackle one of the top five practices chosen for Choosing Wisely by the Society of Hospital Medicine: continuous telemetry monitoring.This activity will review the guideline recommendations and the literature supporting the harms, benefits, and proper utilization of telemetry monitoring.

Rapid Clinical Updates: Updates in Acute Stroke Management

This session is presented by Madison Paul, MD and Emiliya Melkumova, MD. Our moderators are Ebrahim Barkoudah, MD, FACP, MPH, SFHM and Lily Ackermann, MD, ScM, FHM. This talk will discuss high-yield evidence-based answers to questions commonly encountered when taking care of acute stroke patients. This session will be helpful for academic and non-academic hospitalists physicians, physician assistants, nurse practitioners, or anyone else who practices adult hospital medicine. Attendees will learn the appropriate acute treatment of minor stroke and TIA, as well as how to manage anticoagulation in patients with a. fib and breakthrough stroke.

Perioperative Infections and Fever

Perioperative fever has multiple possible etiologies, including infections such as pneumonia, fungal infections, central-line associated infections, and urinary infections. By understanding the typical time frame and common risk factors for each etiology, hospitalists and other perioperative consultants can develop a differential diagnosis for perioperative fever. The following activity will provide a general approach for determining the differential diagnosis in patients with a perioperative fever, as well as an outline of the diagnostic and treatment modalities.

Neurosurgery for the Hospitalist

Successful neurosurgery co-management requires hospitalists to be familiar with the presentations, disease processes, management, and complications of neurosurgical inpatients. The first section of this activity summarizes common inpatient neurosurgical conditions. The second section prepares the reader to recognize and act on neurosurgical emergencies. The remaining four sections of the activity cover blood pressure management in patients with subarachnoid hemorrhage, sodium abnormalities in patients with brain tumors, the use of mannitol vs. dexamethasone, and the perioperative management of anticoagulants, in addition to other topics.
