HQPS Transitions of Care Series
There is significant variability in the facilitation of transitions of care both in the hospital and out of the hospital to home. While there is some consensus regarding evidence-based interventions to successfully facilitate transitions of care and improve patient outcomes, these interventions are not always reliably implemented. This series will feature three transitions of care subtopics that will describe various types of transitions of care with respective best practices. It will also provide learner with a comprehensive overview of best practices and a reliable strategy for implementation for patient transition in, out, and within the hospital. The learner will gain the knowledge and tools to effect change in their institution and individual practice.
