Designed for hospital medicine leaders who have co-management programs or are considering starting one, this program guide outlines key considerations for creating, leading, or participating in a hospitalist co-management model, drawing from a comprehensive review of research and real-world experiences. Included are checklists for creating a hospitalist co-management service, examples of a business plan, a clinical pathway, and a sample interservice agreement, and multiple real-world case examples of hospitalist co-management programs from a variety of practice environments.
