Pdf Summary
The presentation frames hospital throughput as a system-wide capability: moving patients safely to the right care, place, and time—not simply discharging them. Reliable flow creates usable capacity, supports safety and patient experience, and reduces operational and financial strain. Hospitalists are central coordinators, but responsibility is shared across departments.

ED boarding reflects hospital capacity constraints. High occupancy is associated with longer boarding, longer hospital stays, increased risk of harm, and strain on staff. Bottlenecks can arise at any point: admission decisions, staffed beds, consultations, testing, ancillary services, care coordination, or transitions to post-acute and community care. Discharge delays may involve payer requirements, placement availability, patient and family needs, or operational issues such as medications, transport, and bed cleaning.

Recommended actions include mapping demand and constraints by hour and day; aligning staffing with arrivals and staffed-bed capacity; and choosing admission and rounding models deliberately. Teams should use structured interdisciplinary rounds, begin discharge planning at admission, identify barriers early, coordinate consultants and support services, and establish clear escalation pathways for surges and unsafe workloads. The goal is timely clinical decisions and safe transitions—not meeting arbitrary targets such as discharge before noon.

Organizations should track a balanced set of measures covering ED access, admission intervals, inpatient progression, discharge, capacity, outcomes, and workforce impact. Metrics need clear definitions, appropriate comparisons, and balancing measures—for example, assessing readmissions alongside length of stay.

Sustained improvement requires governance: executive sponsorship, system and hospital steering teams, focused workstreams, named owners, transparent data, dated action plans, and reliable follow-up. The key message is to identify the true constraint before adding beds or shifts, coordinate the full patient journey, and make accountability part of daily operations.
