When Rounds Become a Catalyst: How Rebuilding Multidisciplinary Bedside Rounding Unlocked System-Wide Innovation Beyond Its Original Aim

UC San Diego Health rebuilt multidisciplinary bedside FIT rounding after COVID-19 had shifted rounds away from patients. The original goal was to restore patient-centered bedside communication, improve teamwork, and bring back the benefits of shared interdisciplinary rounding. The redesigned process used a standardized communication script involving medicine, nursing, pharmacy, care management, social work, and rehab, with an emphasis on consistent bedside presence, shared mental models, and real-time documentation.<br /><br />As the new rounding model was sustained, it uncovered broader system opportunities beyond the original aim. The project led to several innovations: a multilingual FIT framework to better support patients and families, an EHR-embedded patient deliverable tool, night-shift rounding groups to improve continuity and awareness, discipline-specific subgroup work to tailor data and communication, and structured learner engagement through observation and participation. It also supported real-time data streams through quality dashboards and transition metrics, improving oversight and continuous program development.<br /><br />Overall, rebuilding bedside rounds became a catalyst for system-wide improvement. It strengthened patient engagement, expanded interdisciplinary collaboration, improved day-night coordination, and created a more inclusive, multilingual care framework. The initiative showed that centering care at the bedside can drive innovation well beyond communication alone, generating momentum for ongoing improvements in patient-centered care.
