Utilization of Antiviral Treatment for Hospitalized Patients With Mild-to-Moderate Covid-19 at High Risk for Disease Progression

This study examined hospitalized patients age 50 and older with mild to moderate COVID-19 at high risk for progression, using the CAMC COVID-19 Registry from January 2022 to August 2023. Patients were included if they had a positive COVID-19 test, oxygen saturation above 94% on room air, symptom duration under 5 days, and no recent COVID-19 infection or home oxygen use. Patients receiving tocilizumab or baricitinib were excluded.<br /><br />Among 124 eligible patients, 42 received antiviral treatment and 83 did not. The most common reason for not giving antivirals was mild initial symptoms (74.7% of untreated patients). Other reasons included no documented reason, patient or family refusal, and hepatic or renal disease.<br /><br />The study compared demographics, comorbidities, and outcomes between treated and untreated patients. Groups were generally similar in age, sex, race, vaccination status, smoking, symptom duration, and most comorbid conditions. There were no significant differences in 30-day readmissions, in-hospital deaths, or progression to severe COVID-19. Progression occurred in 7.3% of treated patients and 14.5% of untreated patients, but this difference was not statistically significant.<br /><br />The main conclusion was that antiviral medications were underutilized in hospitalized patients with mild to moderate COVID-19 who were eligible for treatment. The authors suggest that clinicians may be missing opportunities to prevent disease progression, particularly when initial symptoms appear mild.
