Pdf Summary
Northwell Health describes itself as New York State’s largest health care provider and private employer, committed to improving community health through high-quality clinical care, education, research, and care regardless of ability to pay. It operates 23 hospitals, more than 700 outpatient facilities, and includes thousands of affiliated physicians.

The document then presents a hospital medicine project from Long Island Jewish Medical Center on using information technology to reassess diuretic therapy in real time for heart failure patients. Heart failure affects millions of Americans and leads to frequent hospital admissions, longer lengths of stay, higher mortality, and increased costs. To address this, the team created a Microsoft Teams–based tool called “myNora” with a heart failure information card. The card tracks daily standing weights, weight changes, fluid intake, and urine output to help guide intravenous diuretic management.

During interdisciplinary rounds, the heart failure card is reviewed daily. Nurses and physicians confirm that weights are entered, assess volume status, and adjust diuretic therapy immediately if needed. For patients unable to stand, intake and output data are used instead. The project also plans to expand the card to support better discharge guideline-directed medical therapy, including beta blockers, ACEi/ARB/ARNI, MRA, and SGLT2 inhibitors.

Early results showed a decrease in length of stay, with reported improvements over two measured periods. The project aims to improve outcomes, reduce readmissions, and optimize heart failure care through real-time data use.
