Use of Mobility Scoring to Predict Patient Outcomes

This retrospective cohort study examined whether changes in AM-PAC mobility scores could predict hospital outcomes among adult inpatients discharged from a tertiary care center between September and December 2023. The goal was to identify patients at highest risk for prolonged length of stay (LOS) and discharge to a facility, using initial and final AM-PAC “6-Clicks” mobility scores.<br /><br />The study included discharged adults with both initial and follow-up AM-PAC scores, achieving very high documentation compliance (96.6%). Of the cohort, 42.1% showed improving AM-PAC scores during hospitalization. Patients were grouped by change in score (delta AM-PAC), and outcomes were compared between those with improving versus unchanged/worsening mobility.<br /><br />Results did not support the original hypothesis. Patients with unchanged or worsening AM-PAC scores had a lower rate of discharge to a facility than those whose scores improved. In the reported summary table, the improving group had a higher mean age-adjusted facility discharge rate (16.0%) and longer mean LOS (6.63 days) compared with the non-improving group (10.5% facility discharge, 5.10 days LOS). However, after adjustment for demographics and clinical factors, patients with delta AM-PAC 0 still had 1.4 times the odds of discharge to a facility compared with the reference group.<br /><br />The authors concluded that delta AM-PAC alone is not an optimal predictor of discharge disposition or LOS. While AM-PAC remains a useful, well-documented mobility measure, other process or clinical measures should be explored to better identify patients needing resources and discharge planning support.
