Uncovering the Impact of Burns and Corrosion on Mortality: Insights From Two Decades of Data From the CDC Wonder Database (1999-2020)

This study examined U.S. mortality from burns and corrosion using CDC WONDER death certificate data from 1999 to 2020. Using ICD-10 codes T20–T32, the authors calculated age-adjusted mortality rates (AAMR) per 1,000,000 population and assessed trends with joinpoint regression and average annual percent change (AAPC), stratified by age, sex, race/ethnicity, region, and urbanization.<br /><br />Over the two decades, burns and corrosion caused 54,596 deaths in the United States. Overall mortality declined modestly, with AAMR decreasing from 1.38 in 1999 to 1.23 in 2020, showing a significant downward trend (AAPC -0.68). Mortality was much higher in males than females (AAMR 1.68 vs 0.73), though both groups experienced decreases, with a larger decline in men.<br /><br />Disparities were also seen by race/ethnicity. American Indians had the highest mortality rate (2.32), while Asian or Pacific Islanders had the lowest (0.51) and the greatest rate of decline over time. Regionally, the South had the highest AAMR (1.48), whereas the Northeast had the lowest (0.92). Nonmetropolitan areas had higher mortality than metropolitan areas (1.87 vs 1.07). At the state level, Mississippi, Oklahoma, Alaska, Alabama, and South Carolina were among the highest-burden states.<br /><br />Most deaths occurred in medical facilities, followed by decedents’ homes. The authors conclude that although burn- and corrosion-related mortality has decreased in the U.S., important gender, racial, and geographic disparities remain. They recommend targeted prevention and healthcare interventions for high-risk populations and regions.
