This Global Burden of Disease study examined worldwide trends in cocaine use disorders from 1990 to 2019, measuring deaths, disability-adjusted life years (DALYs), incidence, and prevalence across age, sex, region, and socioeconomic levels.<br /><br />Overall, the burden increased substantially. Global deaths rose from 2,355 in 1990 to 12,779 in 2019, a 443% increase. DALYs increased by 114%, while incidence and prevalence rose by 17% and 34%, respectively. However, age-standardized incidence and prevalence rates declined slightly, suggesting that population growth and demographic changes contributed to the absolute rise in burden. In contrast, age-standardized death and DALY rates increased markedly, indicating worsening severity and mortality impact.<br /><br />The largest increases were seen in high Socio-demographic Index (SDI) countries, where deaths increased by 852.5% and DALYs by 111%. Lower-SDI regions showed more stable trends. The findings suggest that factors such as cocaine availability, stress, and cultural influences may be driving the rising burden in wealthier settings.<br /><br />The study concludes that cocaine use disorders are becoming an increasingly important public health problem, especially in high-SDI countries. It emphasizes the need for region-specific prevention and treatment strategies tailored to local social and economic contexts to better reduce harm and address this growing burden.
