Trends in Obesity Across Us Hospitalizations From 2011-2021

This study examined trends in obesity among US adult hospitalizations using the National Inpatient Sample, covering 66.4 million hospitalizations from 2011 to 2021. Obesity was identified using ICD-9/10 codes and defined as BMI ≥30 kg/m². The authors analyzed changes by US Census region (Northeast, Midwest, South, West) and by urbanicity of patients’ residence (metro, medium-small, rural), adjusting for age and sex.<br /><br />Key findings:<br />- Overall, obesity prevalence among hospitalized adults rose from 7.3% in 2011 to 17.0% in 2021, with 12.6% of all hospitalizations including obesity during the study period.<br />- The increase occurred in all regions and all urbanicity categories.<br />- The Midwest had the highest obesity prevalence across all rurality levels and also showed the largest relative increase over time.<br />- The West generally had the lowest prevalence, especially in rural areas; the Northeast tended to have the lowest prevalence in metro areas.<br />- Despite these differences in absolute prevalence, there was no statistically significant difference in the rate of increase across regions.<br />- Among hospitalized patients with obesity, women made up a larger share than men (61.5% vs 38.5%).<br />- The proportion of patients aged 45–64 declined over time, while the shares of Hispanic and Asian/Pacific Islander patients increased and the share of Black patients declined.<br /><br />Conclusion:<br />Hospitalized obesity increased substantially across the US from 2011 to 2021, regardless of region or urbanicity. The Midwest had the greatest burden and growth. The authors suggest more research is needed on outcomes and social drivers to support targeted regional obesity interventions.
