Transcatheter Mitral Valve Procedure Outcomes in Patients With Hocm

This retrospective cross-sectional study examined whether hypertrophic obstructive cardiomyopathy (HOCM) affects outcomes in patients undergoing transcatheter mitral valve procedures (TMVP), using the National Inpatient Sample from January 2017 to December 2019. The investigators compared hospitalized patients who underwent TMVP with HOCM versus those without HOCM, adjusting for demographics, comorbidity burden, and hospital characteristics.<br /><br />The study found that demographic characteristics were generally similar between the two groups. However, patients with HOCM had significantly higher rates of acute kidney injury (AKI) and cardiac arrest after TMVP. Specifically, AKI occurred in 4.00% of HOCM patients versus 0.15% of non-HOCM patients, and cardiac arrest occurred in 4.00% versus 0.56%, respectively. On multivariable analysis, HOCM was associated with much higher odds of AKI, with an odds ratio of 17.53.<br /><br />Unlike some prior reports, mortality was not statistically significant in this study: in-hospital mortality was 0% in the HOCM group versus 2.33% in the non-HOCM group. The authors also note that their findings differ from a 2022 database study that did not find a statistically significant AKI difference in this population.<br /><br />Overall, the results suggest that HOCM is associated with worse renal and cardiac procedural outcomes in patients undergoing TMVP, particularly increased AKI risk. The authors conclude that these findings may help guide risk assessment, but interpretation is limited by the retrospective design and potential confounding factors.
