Time Well Spent: The Influence of on-Call Hospitalist Procedure Teams on Los and the Timeliness of Procedures

This study examined whether launching a hospitalist-run On-call Procedure Team (OPT) improved the timeliness of lumbar punctures (LPs) and affected hospital length of stay (LOS) at a large tertiary urban hospital. Researchers reviewed all LPs performed from April 2022 to April 2024, comparing a pre-OPT period with the period after the OPT began in April 2023. They matched electronic medical record data with a procedure database to measure median LOS and mean time from LP request to completion (R2P).<br /><br />A total of 81 LPs were completed before OPT launch and 84 during the OPT period. Median LOS was 19.0 days before OPT and 21.0 days during OPT, with no significant difference. Mean R2P decreased from 2.8 days pre-OPT to 2.1 days during OPT, but this change was also not statistically significant.<br /><br />Within the OPT period, 63 LPs (75%) were successful on the first attempt. Although LOS was similar between successful and unsuccessful LPs, the time from request to completion was significantly shorter for successful LPs (1.5 days) than for unsuccessful LPs (4.1 days). This suggests that failed or delayed procedure attempts may prolong diagnostic wait times.<br /><br />Overall, the results show a trend toward faster LP completion after the OPT was introduced, though without a clear reduction in LOS. The authors conclude that an organized procedure team may help reduce diagnostic delays, especially when procedures are completed successfully on the first attempt.
