Time Is Not on Our Side: A National Survey of Teaching Time Allocation for Academic Hospitalists

This survey examined how academic hospitalist groups allocate faculty time for inpatient teaching in the face of growing direct-care demands and limited teaching opportunities. The authors surveyed division directors and administrators from the 150 largest Internal Medicine GME training sites; 25 unique hospital medicine divisions responded (17% response rate).<br /><br />Key findings:<br />- On average, 35% of inpatient faculty FTE was dedicated to traditional teaching services, though this varied widely (0–90%).<br />- Clinical faculty spent an average of 10.68 weeks per year on traditional teaching services, with a broad range from 0 to 42 weeks.<br />- About 72% of clinical faculty had some time on traditional teaching services, but only 2 of 25 divisions had a separate faculty group eligible to staff these services.<br /><br />The survey also identified several strategies used to maintain teaching opportunities:<br />- Intentional allocation of teaching time based on faculty preference, strong trainee evaluations, and seniority.<br />- Expansion of opportunities beyond traditional inpatient teaching, including procedure services, direct-care electives, lectures, and faculty development roles.<br />- Creation or expansion of traditional teaching services through reassigning services from other divisions, adding new teaching services, or broadening eligibility.<br /><br />Overall, the study shows that institutions are actively trying to protect teaching time and develop clinician educators, often by combining traditional and non-traditional teaching roles. While the low response rate and focus on large academic centers limit broad generalization, the report offers practical examples for hospital medicine groups facing similar staffing pressures.
