Therapeutic Drugs and Their Deadly Consequences: Exploring Adverse Effects-Related Mortality in the U.S. Using the CDC Wonder Database (1999-2020)

This study examined deaths in U.S. adults aged 25 and older caused by adverse effects of therapeutic drugs from 1999 to 2020, using CDC WONDER death certificate data and ICD-10 codes Y40–Y59. It analyzed age-adjusted mortality rates (AAMRs) and trends by sex, race/ethnicity, region, and urban/rural status using Joinpoint regression.<br /><br />A total of 116,468 deaths were attributed to therapeutic drug adverse effects during the study period. Most deaths occurred in medical facilities (79.2%) or at home (10.43%). Overall mortality declined from 28.96 per 1,000,000 in 1999 to 23.87 in 2020, showing a significant downward trend. However, the decline was not uniform across groups.<br /><br />Men had higher mortality than women, and their mortality increased more steeply over time. Racial disparities were also evident: American Indians or Alaska Natives had the highest AAMR, followed by White, Black, Hispanic, and Asian/Pacific Islander populations. Hispanics showed the largest decline over time. Regionally, the Western U.S. had the highest mortality, and Vermont had the highest state-level rate while Massachusetts had the lowest. Nonmetropolitan areas had higher mortality than metropolitan areas, though both declined over time.<br /><br />The authors conclude that although deaths from therapeutic drug adverse effects have generally decreased, important disparities remain. They recommend better prescribing practices, stronger pharmacovigilance, and targeted prevention efforts for high-risk groups, especially men, American Indians, and residents of nonmetropolitan and western areas.
