The Value of the Observation Unit in Medical Education
Back to course
Pdf Summary
The document argues that hospital observation units have significant but underused educational value for medical trainees. Because observation units are faster paced and handle medically simpler cases, they expose students to frequent discharges, transitions of care, post-procedure monitoring, and opportunities to practice physical exam skills more often than on general medicine wards.

In a 6-week internal medicine inpatient clerkship for physician assistant students, the curriculum included 1 week in a hospitalist-run observation unit, 1 week on an admitter service, and 4 weeks on medicine wards. After the rotation, students provided qualitative feedback. Eight of nine students rated the observation unit positively compared with wards. They reported greater autonomy, more involvement in medical decision-making and discharge planning, and better understanding of the “gray area” between admission and discharge.

Students specifically valued the opportunity to manage simpler patients more independently, practice more exam skills, and better understand post-procedure instructions and disposition decisions. One student described the unit as “faster paced” with “more practice doing physical exams,” while another noted it helped clarify why some patients needed observation rather than full admission.

The authors conclude that observation units can be beneficial learning environments for medical students and other trainees. They suggest that clerkship curricula incorporating 1–2 weeks in observation units may enhance clinical education in both academic and community settings.
