The Readmissions Survey: Quantifying Preventable 30-Day Hospital Readmissions for the General Medicine Service

This project, <strong>“The Readmission Survey: Quantifying Preventable 30-Day Hospital Readmissions for the General Medicine Service,”</strong> examined whether 30-day readmissions on the Duke General Medicine service were preventable and how accurately discharging providers judged preventability. The study was motivated by the fact that hospital readmissions are often used as a quality metric, but the standard 30-day penalization model may be overly simplistic and may miss important patient- and system-level factors. The team’s goals were to encourage providers to reflect on readmissions, estimate the proportion that were truly preventable, identify recurring causes that could guide future interventions, and compare provider judgments with those of the study team. Key findings showed a major difference between provider and reviewer assessments: discharging providers were more likely to label readmissions as preventable than the study team. Out of more than 400 readmissions reviewed, only 10 were judged preventable by the study team. Common preventable themes included patients who needed non-invasive ventilation but were discharged without it, and readmissions for chronic stable disease that may not have required rehospitalization. Planned next steps included expanding the survey to all three Duke hospital sites, using the results to develop targeted interventions for the most common preventable themes, and adapting the survey for other services to create an internal readmission review system.
