The Impact of Social Determinants of Health and Smoking on 30-Day Readmission Rates in Acute Decompensated Heart Failure: A Retrospective Study in a Safety-Net Hospital in the Bronx

This retrospective single-center study examined patients admitted with acute decompensated heart failure (ADHF) and explored how social determinants of health (SDOH) affected 30-day readmission risk. The authors found that patients with any SDOH had nearly twice the odds of readmission compared with those without SDOH (OR 1.98), though this result did not reach statistical significance (p = 0.061).<br /><br />Specific barriers also showed elevated readmission odds, including financial insecurity (OR 1.81), housing instability (OR 1.91), homelessness (OR 2.91), and transportation barriers (OR 2.17), but none of these associations were statistically significant. These findings suggest that social and economic challenges may contribute to worse short-term outcomes after ADHF hospitalization.<br /><br />The background emphasizes that, despite advances in heart failure treatment, morbidity and mortality remain disproportionately high among Black and Hispanic American patients, likely because of environmental and socioeconomic disadvantages. The authors argue that addressing inequities related to SDOH and substance use is essential for improving outcomes.<br /><br />They recommend using Minimally Disruptive Medicine (MDM), an approach that tailors treatment to the patient’s daily life and reduces the burden of treatment. The goal is to support patient-centered, equitable care by making heart failure management more feasible for people facing social hardship.
