The Hidden Cost of Systemic Antibiotics: Exploring Adverse Effects-Related Mortality in the U.S. Through the CDC Wonder Database (1999-2020)

This study examined U.S. deaths related to adverse effects of systemic antibiotics from 1999 to 2020 using CDC WONDER death certificate data and ICD-10 code Y-40. The analysis included adults aged 25 and older and used age-adjusted mortality rates (AAMR) and Joinpoint regression to assess trends by sex, race/ethnicity, and geography.<br /><br />Overall, 3,436 deaths were attributed to antibiotic adverse effects during the study period. Most deaths occurred in medical facilities (71%) or at home (14.6%). The overall AAMR declined from 0.66 deaths per 1,000,000 in 1999 to 0.29 in 2020, with a significant annual decrease (AAPC -3.27).<br /><br />Despite this overall improvement, important disparities remained. Men had slightly higher mortality rates than women, although both groups saw declines over time, with a steeper drop in women. Racial differences were notable: American Indians or Alaska Natives had the highest mortality rate, followed by White individuals, Black individuals, Asians/Pacific Islanders, and Hispanics. White individuals experienced the largest decline over time.<br /><br />Geographically, Vermont had the highest state-level mortality rate, while Louisiana had the lowest. The Western U.S. had the highest regional AAMR, followed by the Midwest, South, and Northeast. Nonmetropolitan areas had higher mortality than metropolitan areas, although metropolitan areas showed a significant downward trend over time.<br /><br />The authors conclude that while mortality from systemic antibiotic adverse effects has decreased over the past two decades, persistent disparities by sex, race, and region highlight the need for better prescribing practices, patient education, and closer monitoring in high-risk populations.
