The First Line of Defense: Hospitalist-Driven Education to Promote Admission Medication Reconiciliation Competency for Trainees

This project evaluated a hospitalist-led educational intervention to improve admission medication reconciliation (AMR) competency among internal medicine residents at Yale New Haven Hospital. The background problem was that medication errors on admission were causing adverse events, and accurate AMR requires both clinical judgment and familiarity with electronic medical record systems, pharmacy databases, and proper workflow. Before this initiative, there was no formal AMR training.<br /><br />The intervention included didactic teaching during a half-day education session for traditional IM residents, plus direct observation and coaching during an admitting rotation called DEFINE-HM. Learners completed pre- and post-assessments measuring knowledge, attitudes, and confidence.<br /><br />Results showed that residents gained knowledge about AMR resources and correct workflows. However, their confidence decreased after the session, likely because they became more aware of the complexity of AMR. Most learners rated AMR as important or critically important, and feedback was highly positive. Resident comments described the session as practical, high-yield, and worthy of being mandatory, especially during the intern-to-resident transition.<br /><br />The authors concluded that hospitalist-driven AMR education improved knowledge, but the drop in confidence suggests that the topic’s complexity was better appreciated after training. Next steps include repeating the sessions, adding hands-on practice, and conducting longitudinal follow-up at 3–6 months to assess retention, reinforcement needs, and interest in practical application sessions.
