The Effect of Sepsis Alert Systems on Mortality: A Systematic Review and Meta Analysis

This systematic review and meta-analysis evaluated whether electronic sepsis alert systems improve outcomes in hospitalized adults or emergency department patients. Although the Surviving Sepsis Campaign recommends sepsis screening, the actual benefit on mortality, length of stay, and time to antibiotics has been unclear.<br /><br />The authors searched multiple databases through June 1, 2024 and included randomized controlled trials of any alert system used to screen adult patients for sepsis. Seven studies involving 3,409 patients were identified. Study quality was assessed with the Cochrane Risk of Bias Tool, and evidence was graded using GRADE. Outcomes were pooled using random-effects meta-analysis.<br /><br />Results showed no significant benefit from sepsis alert systems. The pooled odds ratio for mortality was 0.84 (95% CI, 0.60–1.18), indicating no clear reduction in death. Time to antibiotics was 0.08 hours faster in the alert group, but this difference was not significant. Length of stay was 0.27 days shorter in the alert group, also not significant. Overall, the evidence was rated low due to risk of bias and imprecision.<br /><br />The authors concluded that current randomized evidence is insufficient to recommend sepsis screening with electronic alert systems, and that existing guidelines may need to reconsider their recommendations.
