The Changing Landscape of Post-Kidney Dialysis Mortality: A CDC Wonder Prespective (1999-2020)

This study examined U.S. deaths related to post-kidney dialysis complications from 1999 to 2020 using CDC WONDER death certificate data (ICD-10 code Y84.1) for adults aged 15 and older. Age-adjusted mortality rates (AAMRs) and trends were analyzed overall and by sex, race/ethnicity, region, and urban/rural setting using joinpoint regression.<br /><br />Over the 22-year period, 22,250 deaths were attributed to dialysis complications. Most deaths occurred in medical facilities (70.0%), while 17.7% occurred at home. The overall AAMR rose nearly sixfold, from 0.27 per 100,000 in 1999 to 1.61 in 2020, with an average annual percent increase (AAPC) of 8.5% (p=0.01).<br /><br />Men had higher mortality than women (0.46 vs. 0.31), and their increase over time was slightly steeper. Clear racial disparities were identified: non-Hispanic Blacks had the highest mortality rate, followed by non-Hispanic American Indians, Hispanics, non-Hispanic Asians/Pacific Islanders, and non-Hispanic Whites. Hispanics experienced the fastest increase over time. Geographically, the District of Columbia had the highest state-level AAMR, while Montana had the lowest. The South had the highest regional mortality, and rural areas had slightly higher rates than urban areas.<br /><br />Overall, the findings show a substantial and worsening burden of dialysis-related mortality, with marked disparities by race, sex, and geography. The authors call for urgent targeted interventions, improved dialysis practices, and better healthcare professional education to reduce preventable deaths.
