Temporal Trends and Disparities Related to Intra-Hepatic Cholangiocarcinoma Amond Adults Living in United States Between 1999 and 2020; A CDC Wonder Database Study.

This CDC WONDER study examined intrahepatic cholangiocarcinoma (IHCC)-related deaths among U.S. adults aged 25 years and older from 1999 to 2020. Using age-adjusted mortality rates (AAMR) and annual percentage change (APC), the authors analyzed trends by age, sex, race, region, urbanization, and place of death.<br /><br />A total of 113,350 adults died from IHCC during the study period. Overall mortality increased steadily, with the AAMR rising from 1.5 per 100,000 in 1999 to 3.1 in 2020 (APC 3.55). Mortality increased with age and was highest among adults aged 85 years and older (AAMR 12.2). Men had slightly higher mortality than women throughout the study period.<br /><br />Racial disparities were evident. Non-Hispanic Asian and Pacific Islanders had the highest overall mortality (AAMR 3.2), followed by Non-Hispanic American Indians/Alaska Natives and Hispanics/Latinos (both 2.7), and Non-Hispanic Whites and Non-Hispanic Blacks/African Americans (both 2.3). Regionally, mortality was highest in the Northeast (AAMR 2.6), followed by the Midwest and West (2.5), and the South (2.1). Metropolitan areas also had higher mortality than non-metropolitan areas (2.4 vs 2.1). New England showed the highest state-level mortality.<br /><br />The study concludes that IHCC mortality has risen significantly in the U.S. over the past two decades, with marked disparities by age, sex, race, and geography. These findings highlight the need for earlier diagnosis, improved surveillance, and targeted therapies to address increasing mortality and reduce inequities.
