Temporal Trends, Predictors, and Outcomes of Catheter-Associated Urinary Tract Infections: A Retrospective Analysis From the National Inpatient Sample Database (2011-2018)

This retrospective study examined temporal trends, predictors, and outcomes of catheter-associated urinary tract infections (CAUTI) using the National Inpatient Sample from 2011–2018. Among 58,066,154 sampled patients, 137,857 were diagnosed with CAUTI and 311,305 had urinary catheter placement identified by procedure codes.<br /><br />The authors used ICD-9/ICD-10 codes to identify cases and multivariate hierarchical logistic regression to evaluate associations between patient characteristics, comorbidities, and mortality. Temporal trends were assessed using Pearson correlations.<br /><br />Key findings included notable socioeconomic disparities and outcome differences. Female patients had a significantly higher risk of mortality compared with males. Patients with transfer status also had increased mortality risk. Differences in payer status were associated with higher mortality compared with Medicare patients.<br /><br />The study also found a downward trend in urinary catheter placement over time, which may reflect national efforts to reduce CAUTI risk. In addition, mean length of stay improved over the study period, suggesting possible gains in quality of care. Mean total charges and catheter placement patterns were also tracked across the years.<br /><br />Overall, the study highlights that CAUTI remains an important cause of morbidity, with mortality risk influenced by demographic and access-related factors. The observed decline in catheter use and improved hospital stay metrics suggest progress in prevention and management, though disparities persist.
