Teaching Hospital Medicine Skills to Medical Students: A Cross Cover Simulation

This poster describes a 1.5-hour cross-cover simulation designed for graduating medical students to improve readiness for internal medicine residency. Recognizing that cross-cover care is common but rarely formally taught, the authors created a small-group exercise using secure text messaging rather than a traditional simulation center.<br /><br />In the session, students were divided into groups of ten and led by hospital medicine faculty. A remote faculty facilitator sent mock page texts every 5–7 minutes, prompting students to triage and respond to 10 common overnight cross-cover scenarios. The cases included issues such as hypotension, oxygen desaturation, fever, and pain management requests.<br /><br />The activity was well received. Of 68 students, 64 rated the session as highly relevant to future practice. Most participants reported feeling much more comfortable managing cross-cover situations after the simulation, with 63% selecting “much more comfortable” and the remaining students selecting “somewhat more comfortable.”<br /><br />Student comments were strongly positive, describing the experience as concrete, helpful, and valuable preparation for intern year.<br /><br />Overall, the project demonstrates a scalable, cost-effective way to teach practical hospital medicine skills to medical students. Because it relies on secure messaging and faculty facilitation rather than specialized simulation equipment, it can be implemented in a variety of teaching settings.
