Systemic Factors Associated With Physician Burnout Among Hospitalists: An Anonymous Survey Study

This anonymous survey study examined systemic factors associated with physician burnout, professional fulfillment, and resilience among hospitalists in a large New York health system during the COVID-19 era. The goal was to identify modifiable workplace factors, such as schedule predictability, leadership approachability, and electronic health record (EHR) efficiency, that may influence burnout and well-being.<br /><br />A voluntary email survey was sent to all hospitalists between November 3 and November 24, 2020. Sixty-six hospitalists responded. Most respondents were ages 30–39, married or in a domestic partnership, and female. Nearly half of respondents (46.97%) reported low EHR efficiency, 22.73% reported low satisfaction with schedule predictability, and 12.12% reported low satisfaction with leadership approachability. Low job satisfaction was reported by 10.61%.<br /><br />Key findings showed that satisfaction with schedule predictability was associated with higher personal accomplishment, professional fulfillment, and resilience. Satisfaction with leadership approachability had the strongest and broadest association: it was linked to lower emotional exhaustion, depersonalization, and work exhaustion, as well as higher personal accomplishment and professional fulfillment. Satisfaction with EHR efficiency was only associated with increased resilience.<br /><br />The authors concluded that schedule flexibility and approachable leadership play important roles in reducing burnout and improving professional fulfillment among hospitalists. They suggest that future efforts to reduce moral distress and physician burnout should focus on system-level interventions rather than individual coping alone.
