Synchronous Lung Neoplasms of Divergent Histologies Alongside Concurrent Prostate Adenocarcinoma

A 74-year-old heavy smoker with advanced stage III prostate adenocarcinoma treated with androgen deprivation therapy and radiotherapy later developed exertional dyspnea during surveillance. Chest CT showed obstructing lesions in both upper lobes, and PET/CT demonstrated hypermetabolic bilateral lung masses with an endobronchial component on the left. Bronchoscopy with biopsy revealed two different primary lung cancers: poorly differentiated adenocarcinoma in the right lung and squamous cell carcinoma in the left lung.<br /><br />This created a rare scenario of three synchronous malignancies: prostate cancer plus two distinct lung cancers with different histologies. Radiation provided temporary disease control, but after two years the patient deteriorated clinically with progressive lung masses, pleural nodules, and bone lesions. Because biopsy was no longer feasible, pleural fluid analysis confirmed metastatic lung cancer, and care shifted to palliation.<br /><br />The report emphasizes that multiple primary malignancies are uncommon but increasingly recognized, likely due to improved cancer screening and longer survival after initial cancer treatment. Proposed contributors include shared risk factors such as tobacco use, genetic predisposition, prior chemo-radiotherapy, and ongoing surveillance. The case is notable as the first reported instance of simultaneous bilateral lung cancers of different histologic types occurring alongside prostate cancer.
