Substance Use in Heart Failure and Burden of Treatment

This study examined how alcohol, tobacco, and other substance use relate to social determinants of health (SDoH) and burden of treatment (BoT) among patients hospitalized for congestive heart failure (CHF) at a safety-net public hospital in the South Bronx. Researchers enrolled 159 patients admitted with CHF exacerbation between April and November 2022 and used the PETS tool to assess treatment burden, along with a validated survey of SDoH such as bills, food insecurity, housing, benefits, immigration, social isolation, and homelessness. Baseline clinical data were collected from charts.<br /><br />The patient population had a mean age of 62 years, was 50.3% male, and 66.4% had heart failure with reduced ejection fraction (HFrEF). Substance use was common: 30.8% reported alcohol use, 23.3% substance use, and 29.6% tobacco use.<br /><br />Key findings showed that alcohol, substance, and tobacco use were all associated with social isolation. Alcohol use was also associated with needing help with social security benefits and housing insecurity, as well as greater difficulty managing medications and attending medical appointments. Substance use was linked to difficulty paying bills, needing benefits assistance, and problems with healthcare services such as communication and access. Tobacco use showed a significant association with social isolation and difficulty keeping medical appointments, with possible links to food insecurity and benefits assistance.<br /><br />Overall, the study highlights that in urban, underserved HF populations, substance use is closely tied to social hardship and higher treatment burden. The authors conclude that improving heart failure outcomes requires addressing these social and behavioral challenges through tailored, resource-aligned care strategies to reduce health disparities.
