Structured Interdisciplinary Bedside Rounds in the Oncology Ward

This project piloted Structured Interdisciplinary Bedside Rounds (SIBR) on a high-complexity oncology unit at Huntington Hospital to improve communication, teamwork, and patient outcomes. Oncology patients often receive care from multiple consultants, increasing the risk of fragmented care and loss of trust. The SIBR model brought together the hospitalist, primary nurse, nurse manager, case manager, and social worker each morning for bedside rounds using open-ended questions to better understand patient concerns and build a shared care plan.<br /><br />The team rounded on an average of 14 patients over 90 minutes, with additional interdisciplinary rounds for patients not seen at bedside. A project manager tracked punctuality, attendance, time, and patient volume, and the team reviewed performance daily for rapid feedback. Outcomes were compared for six months before and after implementation using HCAHPS scores and operational measures.<br /><br />Results showed improved patient experience scores, especially in communication. “Communication with Doctors” improved from 81.8% to 84.3%, and “Communication with Nurses” improved from 83.3% to 87.6%. Operational outcomes were mixed: falls dropped significantly from 5 to 1, likely because the team identified bed alarm issues, pain, and delirium earlier. However, length of stay rose slightly from 4.8 to 5.2 days, and 30-day readmissions increased marginally from 15.1% to 15.4%. Discharges by 2:00 pm improved modestly from 21% to 23.3%.<br /><br />Overall, the SIBR approach improved communication, reduced interruptions, increased staff engagement, and may have contributed to fewer falls. The authors suggest further study to evaluate effects on rapid response events, length of stay, and readmissions.
