Strengthening the Hospital Medicine Workforce: Results From an Np/Pa Transition to Practice Initiative

This report describes a 12-month transition-to-practice (TTP) program designed to support new and experienced nurse practitioners and physician assistants entering hospital medicine. Developed using ABIM and SHM frameworks, the curriculum included weekly 90-minute sessions covering foundational medicine, organ-system topics, professional development, and guided readings. Sessions were offered live and asynchronously to accommodate varied schedules.<br /><br />Evaluation used Kirkpatrick Levels 1–2 and included participant satisfaction surveys, pre/post NBME-style knowledge tests, and a 7-point self-reported confidence scale measured at baseline, mid-program, and completion. Participants represented a heterogeneous group of APPs across 31 medical centers in 18 states, with 65% PAs and 35% NPs.<br /><br />Results showed high satisfaction with the webinars and significant gains in knowledge across all assessed domains (paired t-test, p<0.001). Confidence also improved over time, with the largest increase in “comfort in the role” (mean 4.2 to 6.1). Importantly, post-program knowledge was not associated with live attendance, suggesting that asynchronous access was also effective for learning.<br /><br />Overall, the program appears to have met the educational needs of APPs entering hospital medicine by providing relevant, clinically focused training. The findings support both objective and subjective learning gains. However, the authors note limitations, including the absence of a control group and possible selection bias, which limit generalizability.
