Steroid Salvation or Stress ? Causative Role of Corticosteroids in Scleroderma Renal Crisis

This case describes a 37-year-old woman with systemic sclerosis who developed new severe hypertension after recent exposure to medium-to-high-dose corticosteroids. On admission, she had tachycardia and markedly elevated blood pressure, and she met criteria for scleroderma renal crisis (SRC). Initial treatment with captopril did not improve her condition. Her renal function worsened, leading to oliguria, fluid overload, and progressive anemia and thrombocytopenia with schistocytes, elevated LDH, and low haptoglobin, consistent with microangiopathic hemolytic anemia from SRC. Thrombotic thrombocytopenic purpura was excluded because ADAMTS13 was normal. Despite renal replacement therapy, she deteriorated further, became hypotensive, required vasopressors, and died of sepsis.<br /><br />The report emphasizes that corticosteroid use is a major risk factor for SRC, especially in patients with early diffuse skin disease and recent high-dose steroid exposure. In this patient, both risk factors were present, placing her at particularly high risk. The authors note that SRC is a medical emergency defined by rapidly progressive renal failure with or without hypertension.<br /><br />Their conclusion is that clinicians need a clearer, urgency-based criterion to identify patients at risk for SRC before prescribing corticosteroids in systemic sclerosis. When corticosteroids are unavoidable, prednisone should be limited to 15 mg/day for the shortest possible duration. High-risk patients should be closely monitored with daily home blood pressure checks and periodic creatinine and urine protein testing. The goal is to reduce potentially iatrogenic cases of SRC and improve early referral for steroid-sparing management.
