This case describes idiopathic plantar eccrine hidradenitis, a rare, self-limited inflammatory skin condition seen mainly in children and adolescents, often in warm months or after prolonged moisture exposure. It is associated with hyperhidrosis and is thought to result from obstruction of eccrine sweat glands.<br /><br />A 12-year-old boy presented with bilateral foot swelling, pain, fever, abdominal pain, nausea, and vomiting after spending hours in wet shoes. He had a history of hyperhidrosis. On exam, he had bilateral plantar erythema and mild edema, along with conjunctival injection and mild abdominal tenderness. Initial labs showed leukocytosis with neutrophil predominance and elevated inflammatory markers, including CRP 7.8.<br /><br />Because of fever, foot swelling, abdominal symptoms, and conjunctivitis, clinicians considered Kawasaki disease and MIS-C, and cellulitis was also suspected. He was initially treated with antibiotics and admitted. However, as his symptoms evolved overnight, he developed a characteristic bilateral blanching erythematous plantar rash with patches and macules, supporting the diagnosis of plantar eccrine hidradenitis. His condition improved with supportive care using acetaminophen and ibuprofen, and symptoms resolved within a few days.<br /><br />The report emphasizes that plantar eccrine hidradenitis can mimic cellulitis and other serious inflammatory or infectious diseases. Recognizing the typical history and physical findings can prevent unnecessary testing, hospitalization, and antibiotic use.
