Sociodemographic Differences and Outcomes of Patients Admitted During the Covid-19

This study examined how COVID-19 affected outcomes in patients hospitalized with acute myocardial infarction (AMI) using 2020 U.S. National Inpatient Sample data. Among 567,990 AMI admissions, 1,154 patients had concomitant COVID-19 and were propensity-matched to 109,990 non-COVID AMI patients.<br /><br />The two groups were similar in sex distribution, age, and overall comorbidity burden. However, AMI patients with COVID-19 had lower rates of several traditional cardiovascular risk factors, including dyslipidemia, peripheral vascular disease, smoking, hypertension, and carotid artery disease.<br /><br />Despite having fewer baseline risk factors, the COVID-19 + AMI group had significantly worse in-hospital outcomes. COVID-19 was associated with higher hospital mortality, greater risk of cardiogenic shock, cardiac arrest, and respiratory failure, and longer hospital stays. The study also found increased hospitalization burden in this subgroup.<br /><br />The authors conclude that COVID-19 substantially worsens outcomes in patients with AMI, even though these patients may have fewer conventional cardiac risk factors. They suggest that clinicians should maintain a high index of suspicion and use rapid, protocol-driven management for AMI patients complicated by COVID-19 to improve outcomes.
