This project examined how social determinants of health (SDOH) influence patient experience survey participation and HCAHPS scores within Northwell Health. The patient experience committee used an HCAHPS analytics tool to analyze 28,293 survey responses over 12 months, grouping results by age, education level, race, and primary language. These findings were then compared with the demographics of the broader patient population served during the same period (N=250,356).<br /><br />The analysis showed a clear disparity in survey engagement by race. White patients made up 77% of survey respondents, compared with 54% of the overall patient population. In contrast, Black patients represented 9% of respondents but 16% of the population served, Asian patients 7% of respondents and 8% of the population, and multiracial patients were notably underrepresented among respondents. This suggests higher survey return and engagement among White patients, while minority populations were less likely to respond.<br /><br />The findings highlight the importance of SDOH in shaping patient engagement and the need for targeted outreach to ensure more equitable participation across all groups. The study emphasizes that better understanding of these factors can help healthcare providers identify patients who may need stronger follow-up and communication efforts.<br /><br />Next steps include sharing the data with site leadership, developing site-specific action plans, and implementing interventions to improve engagement among minority populations. The committee also aims to support a more inclusive and equitable healthcare culture by ensuring diverse voices are represented in decision-making and quality improvement efforts.
