Social Determinants and Burden of Treatment in Heart Failure Patients

This ongoing prospective study examined how social determinants of health (SDoH) and health-related quality of life (HRQoL) relate to burden of treatment (BoT) in hospitalized heart failure (HF) patients. BoT refers to the workload patients must manage because of their healthcare and how this may overwhelm their capacity for self-care.<br /><br />A total of 265 consenting patients were enrolled from 294 screened (90% response rate). The sample was largely male (66%), with a mean age of 63.7 years. Nearly half were Hispanic (50%) and nearly half Black (48%). Many had substantial comorbidity, including hypertension (90%), diabetes (52%), and chronic kidney disease (40%). Mean household income was about $27,000/year, and 45% had a high school education level.<br /><br />Participants completed surveys measuring SDoH, BoT using the Patient Experience with Treatment and Self-management (PETS) tool, and HRQoL using EQ-5D. Descriptive analyses showed substantial burden across multiple BoT domains, especially medical expenses, role and social activity limitations, difficulty with healthcare services, and medical information/health monitoring.<br /><br />Key findings were that age, gender, race, and substance abuse were not significantly associated with BoT severity. In contrast, higher levels of SDoH were significantly associated with greater burden in seven of ten BoT domains, and worse HRQoL was associated with increased burden in six of ten domains. Specific associations included:<br />- HFpEF linked to greater role/social activity burden<br />- Less than high school education linked to poorer understanding of medical information<br />- Lower income linked to greater medical expense burden<br />- Poorer HRQoL linked to more fatigue and difficulty with appointments<br /><br />Overall, the study suggests that social disadvantage and poorer quality of life meaningfully increase treatment burden in HF, highlighting the need for interventions that address SDoH to reduce disparities and improve outcomes.
