Small Doses, Big Impact: A Longitudinal, Multi-Site Anti-Racism Curriculum Transforms Knowledge, Attitudes, and Behaviors of Hospitalist Educators

This prospective multicenter cohort study evaluated a longitudinal microlearning anti-racism curriculum, “5-Minute Moment on Racial Justice,” for hospitalist educators at six U.S. practice sites. The curriculum used 10 brief, case-based sessions delivered monthly during division meetings from February 2024 to March 2025 and focused on clinical decision-making in adult and pediatric hospital medicine.<br /><br />Participants completed a baseline survey and then quarterly follow-up surveys across three post-intervention time points. The study assessed changes in knowledge, attitudes, and behaviors related to racial justice in clinical teaching. Analyses used descriptive statistics and generalized mixed-effects models to estimate changes over time.<br /><br />Sixty-three participants enrolled, and 59 completed at least one post-intervention survey. By the end of the curriculum, participants reported substantially improved comfort discussing anti-racism with patients, colleagues, and learners. Reported odds of discussing anti-racism increased markedly, with especially large gains for conversations with patients. Participants also showed increased self-perceived skill in navigating anti-racism discussions and reported more changes in clinical decision-making and teaching practice.<br /><br />Overall, 45% of participants said the curriculum changed their clinical or teaching behavior. The intervention was associated with strong positive effects, including increased confidence across all 10 clinical case topics and improved willingness to apply anti-racism concepts in practice.<br /><br />The authors conclude that a brief, repeated microlearning curriculum can meaningfully improve hospitalists’ anti-racism knowledge, attitudes, and behaviors. Monthly 10-minute sessions were linked to sustained gains in comfort, skill, and practice change, suggesting that small, longitudinal educational interventions can have a large impact.
