Sle Associated With Higher Mortality and Aki in Hospitalized-Covid-19 Patients

This retrospective cohort study used 2020 National Inpatient Sample data to examine outcomes of hospitalized COVID-19 patients with and without systemic lupus erythematosus (SLE). Among 1,050,040 COVID-19 hospitalizations, 5,745 (0.54%) had a co-diagnosis of SLE.<br /><br />Patients with SLE were more likely to be younger, female, and Black or Hispanic compared with non-SLE patients. They also had different comorbidity patterns, including higher rates of chronic pulmonary disease, obesity, renal failure, heart failure, anemia, hypothyroidism, and drug abuse, but lower rates of diabetes, dyslipidemia, atrial fibrillation, smoking, and ischemic heart disease.<br /><br />After adjustment for confounders, SLE was associated with worse renal and mortality outcomes. Specifically, SLE patients had higher odds of acute kidney injury requiring dialysis (adjusted odds ratio [aOR] 1.55, 95% CI 1.06–2.26, p=0.024) and higher in-hospital mortality (aOR 1.25, 95% CI 1.01–1.55, p=0.036). The increased mortality risk was especially notable in the 18–44 age group. Other severe outcomes such as invasive mechanical ventilation, pressor use, and cardiogenic shock were not significantly different.<br /><br />The study concludes that hospitalized COVID-19 patients with SLE represent a vulnerable population with elevated risk of dialysis-requiring acute kidney injury and death, underscoring the need for closer monitoring and individualized management. Key limitations include possible coding misclassification, lack of detail on disease severity and medication use, and absence of long-term follow-up.
