Shining a Light on Nights: Optimizing Academic Night Float Rotations

This project evaluated a formal teaching curriculum for internal medicine residents on night float rotations, addressing the problem that residents often miss conferences and other daytime educational activities. At Mount Sinai, nocturnists led one-hour nightly teaching sessions for residents. These sessions focused on overnight admissions, assessment and plan development, and questions about cross-covering patients. Resident satisfaction was measured using an anonymous nine-question survey.<br /><br />Thirty residents responded out of 105 eligible residents who had completed at least one night float rotation, for a 29% response rate. Most respondents viewed the sessions positively: 43% rated them “Somewhat Useful,” 37% “Useful,” and 10% “Very Useful.” Residents also preferred a flexible teaching schedule rather than a fixed one. Free-response comments provided useful feedback that helped guide changes to later night float blocks.<br /><br />The authors conclude that structured overnight teaching can improve the educational value of night float rotations. They also note that anonymous GME survey results improved in the area of night supervision, rising from the 65th to the 90th percentile. Key limitations included the low response rate and the absence of a randomized control group.
