Sharing Pearls, Shaping Leaders: A Novel Peer Education and Resident Leadership (Pearl) Rotation for Internal Medicine Pgy-3 Residents on Inpatient Wards

The PEARL (Peer Education and Resident Leadership) rotation was designed to strengthen teaching and leadership skills in PGY-3 internal medicine residents while improving the transition into supervisory roles for PGY-2 residents and supporting PGY-1 residents during internship. The program addressed a common challenge in residency training: senior residents are expected to teach and lead, but heavy clinical demands often leave little time for structured development.<br /><br />In this rotation, an additional non-clinical PGY-3 “teaching senior” was embedded on traditional ward teams. These residents provided near-peer teaching during rounds, led structured chalk talks, mentored PGY-2 residents, observed interns, and offered feedback on clinical skills. They also participated in simulated attending-level decision-making with support from weekly clinician-educator-led teaching sessions.<br /><br />The program was evaluated using pre- and post-rotation surveys. PGY-3 residents reported increased confidence in teaching, rising from 3.7 to 4.6 on a 5-point scale, and improved confidence in observing and giving feedback on clinical skills, rising from 3.7 to 4.4. PGY-2 residents reported improved learning experiences (4.4/5) and a smoother transition into senior roles (4.4/5), without feeling a loss of leadership autonomy (4.2/5). PGY-1 residents reported enhanced learning experiences with the added PGY-3 teaching senior (4.8/5).<br /><br />Overall, the PEARL rotation improved education and professional development across all postgraduate years. It gave PGY-3 residents protected time to build teaching and leadership skills, helped PGY-2 residents gain confidence in supervisory roles, and provided PGY-1 residents with better educational and logistical support. The authors conclude that PEARL is a sustainable and replicable model for residency programs.
