Sexual Harassment Encountered and Reporting Amang Medical Residency in Japan: Nationwide Cross-Sectional Survey

This nationwide cross-sectional survey examined sexual harassment among first- and second-year postgraduate residents in Japan who took the GM-ITE in 2023. Among 4,459 respondents (67.3% men; 89.2% under 30), 9.6% reported experiencing some form of sexual harassment. Rates were much higher among women than men (21.5% vs. 3.8%). The most common forms were gender harassment (6.5%) and unwanted sexual attention (4.6%), with sexual coercion rare (0.3%).<br /><br />Perpetrators were most often men doctors (72.4%), followed by men patients (31.9%). Women residents more often reported harassment from men patients, while men residents more often reported women doctors or women co-medical staff, though overall perpetrators were predominantly male. About half of respondents (51.1%) knew the reporting structure, but only 11.7% of those who experienced harassment actually reported it. Most victims said they would not report the incident (72.6%).<br /><br />The authors conclude that sexual harassment among Japanese residents is a significant problem, especially for women, and that underreporting is severe. They suggest that reducing harassment will require better reporting systems, improved training environments, and stronger institutional support. Future research should explore why victims do not report incidents, including fear of retaliation, negative workplace consequences, and being treated differently after reporting.
