Severe Thrombocytopenia in a Case of Lemierre's Syndrome

His <strong>PLASMIC score was 6</strong>, suggesting high risk for severe ADAMTS13 deficiency, so plasma exchange and steroids were started. However, his <strong>ADAMTS13 level was only mildly reduced (57%)</strong>, making TTP unlikely. Blood cultures grew <strong>Fusobacterium necrophorum</strong>, and with continued antibiotic therapy, his <strong>platelet count, white blood cell count, and anemia improved</strong>, supporting the infection as the underlying cause. The authors conclude that the most likely explanation for the severe thrombocytopenia and hemolysis was <strong>an intrinsic effect of Fusobacterium</strong>, possibly through platelet aggregation and hemolysin production.
