Sepsis Readmission Prediction: Pilot Study on Social Determinants of Health

This pilot prospective cohort study examined whether social determinants of health (SDOH) data could be feasibly collected and whether it was associated with 30-day hospital readmission among adult sepsis patients at three Duke University–affiliated hospitals in North Carolina from February to July 2023. Using a validated pre-built SDOH questionnaire in the EPIC electronic health record, the study enrolled 173 patients diagnosed with sepsis during hospitalization.<br /><br />The most common sources of sepsis were pneumonia (30.1%), intraabdominal infection (23.1%), skin and soft tissue infection (20.2%), genitourinary infection (16.8%), and osteomyelitis (12.1%). Common clinical features included ICU admission in 38% of patients, infectious disease consultation in 57%, and a median of 13 antimicrobial treatment days. At discharge, the average EPIC readmission risk score was higher in patients who were later readmitted (27.9 vs 23.6), and readmitted patients also had a higher Charlson Comorbidity Index (3.9 vs 2.6).<br /><br />Overall, the study found a 30-day readmission rate of 28.9% and a 30-day mortality rate of 7.5%. The authors note that readmission risk appeared to be strongly influenced by chronic comorbidities and potentially by SDOH variables. The pilot study supports the feasibility of collecting SDOH information in sepsis patients and suggests that larger studies are needed to better understand how these factors contribute to readmission risk.
