Sensitivity and Specificity of Point-of-Care Ultrasound (Pocus) for Diagnosing Cardiovascular Diseases

This umbrella meta-analysis evaluated how accurately point-of-care ultrasound (POCUS) diagnoses and predicts outcomes in cardiovascular conditions, specifically cardiac arrest and deep vein thrombosis (DVT), compared with gold-standard methods. The authors noted that while bedside ultrasound is well established for traumatic cardiac injury and aortic aneurysm, evidence is limited for other cardiovascular uses.<br /><br />Using a PRISMA-based search of PubMed through 2021, they identified three meta-analyses covering 33 studies and 4,216 patients: two meta-analyses on cardiac arrest (25 studies, 3,181 patients) and one on DVT (8 studies, 1,035 patients). Data were pooled using random-effects models in Review Manager 5.3 and OpenMetaAnalyst, with heterogeneity assessed and ROC curves generated.<br /><br />Results showed a pooled sensitivity of 81.7% and a pooled specificity of 90.1% for POCUS in accurately diagnosing cardiovascular disease compared with reference standards. However, heterogeneity was high, especially for specificity, indicating considerable variation across studies.<br /><br />The authors concluded that POCUS appears to have good overall diagnostic performance for these cardiovascular conditions and may help in both diagnosis and prognosis prediction. They also emphasized the need for more research on additional cardiovascular diseases to strengthen the evidence base.
