Seated Physician Rounds and Effective Communication for Improving Patient Satisfaction

This quality improvement project evaluated whether seated physician rounds, combined with effective communication training, could improve patient satisfaction on a medical inpatient unit. The team implemented the intervention on one floor (B5) using the AIDET communication framework (Acknowledge, Introduce, Duration, Explain, Thank you). Physicians received didactic sessions on AIDET, chairs were placed in patient rooms, and “confirmatory rounds” were used to monitor adherence. Patient satisfaction was measured using HCAHPS survey scores, with comparisons made to prior months and to other inpatient medical units.<br /><br />The goal was to achieve a 25% increase in HCAHPS scores across four communication-related domains within three months. Early results showed marked improvement on the intervention unit. From May to July 2022, the percentile ranks increased substantially in all four domains: communication with doctors (14% to 83% to 99%), courtesy/respect (25% to 61% to 99%), listening carefully (3% to 66% to 99%), and explaining in understandable terms (35% to 94% to 99%). In contrast, other medical inpatient units did not show similar gains.<br /><br />The authors conclude that bedside rounds conducted while seated, paired with structured communication training, can significantly improve patient satisfaction and may enhance overall quality of care. Based on the positive early outcomes, the protocol was expanded to other inpatient medical units.
