Safety of Laa Closure by Minimally Invasive Procedure vs Oacs in Patients With Atrial Fibrillation - An Umbrella Meta-Analysis

This umbrella meta-analysis evaluated the safety and effectiveness of minimally invasive left atrial appendage (LAA) closure devices, specifically Watchman and Amulet, compared with oral anticoagulants (OACs) in patients with non-valvular atrial fibrillation. The review followed PRISMA methods and used published meta-analyses identified through PubMed. Outcomes of interest included mortality, stroke, and bleeding, and pooled odds ratios were calculated using a random-effects model.<br /><br />Nineteen studies were included, with 12 providing usable outcome data. Overall, LAA closure was associated with better safety and clinical outcomes than OACs. The devices showed a significant reduction in mortality, including all-cause mortality and cardiovascular mortality. Specifically, pooled results suggested lower odds of death with LAA closure. Major bleeding was also significantly reduced compared with anticoagulant therapy. The risk of stroke was lower as well, though this finding was less definitive and showed considerable heterogeneity.<br /><br />The analysis found that Watchman and Amulet devices may offer a safer alternative to oral anticoagulation in selected atrial fibrillation patients. However, the authors noted that the evidence has moderate risk of bias and that more prospective studies and clinical trials are needed. Future research should help identify which patient groups benefit most from minimally invasive LAA closure and improve risk stratification for treatment selection.
