Running a Code: Cardiac Arrest for the Rising Senior

This project evaluated an interactive ACLS leadership session for rising PGY-2 internal medicine residents, addressing a common gap between prior training and low confidence in leading in-hospital cardiac arrests.<br /><br />Residents completed pre-tests measuring confidence (5-point Likert scale) and ACLS knowledge (multiple-choice questions). They then participated in a 60-minute small-group, case-based lecture using simulated cardiac arrest scenarios. Post-tests assessed immediate improvement, and a 6-month follow-up survey measured retention.<br /><br />Results showed significant gains in both confidence and knowledge immediately after the session (p < .001 for both). Male participants demonstrated greater confidence improvement than female participants (p = .005), while knowledge gains did not differ by gender. At 6 months, confidence remained above baseline (p = .01) but declined from immediate post-session levels (p = .005). Knowledge was slightly higher than baseline but did not reach statistical significance (p = .07).<br /><br />The study included 30 participants at baseline, but only 8 completed the 6-month follow-up, limiting long-term conclusions. Additional limitations included small sample size and inconsistent survey lengths across timepoints.<br /><br />Overall, the findings suggest that focused, interactive, activity-based ACLS leadership teaching can significantly improve residents’ confidence and knowledge, with some persistence over time. However, the results also indicate that multiple interactive sessions and confidence-building strategies may be needed to sustain gains, especially among women. The authors plan to integrate the module into an ICU elective with simulation and real-time code response experience.
