Rotation X is a 5-day quality-improvement curriculum created by the Upstate Medical University Hospital Department of Medicine to align residency training with Vizient quality domains and ACGME requirements for patient safety and quality improvement education.<br /><br />The program was developed in response to the difficulty of engaging residents in institutional quality goals. It introduces residents, especially PGY-1 and PGY-3 trainees, to key hospital quality metrics and shows how their clinical documentation and daily practices affect outcomes such as mortality, DRG reimbursement, and denials.<br /><br />During the rotation, the departmental QI team guides residents through the hospital’s quality goals, patient safety indicators, and organizational scorecard. Residents learn about the Observed/Expected Mortality index, Vizient mortality measures, and the six Vizient domains: mortality, efficiency, effectiveness, equity, patient experience, and patient safety. They also participate in sessions on documentation improvement, root cause analysis, Tableau-based quality review, and education on QI principles.<br /><br />The rotation includes practical, interdisciplinary experiences with pharmacy, infection control, and nursing. Residents perform medication reconciliation with pharmacy, learn about CAUTI and CLABSI prevention, round on patients to reduce unnecessary lines, and receive instruction on handoff quality and communication through RN/MD communication sessions and supervision. Patient experience topics, such as voice of the customer, are also included.<br /><br />The curriculum has strengthened resident engagement in quality improvement and has helped generate multiple resident-led QI projects. Overall, Rotation X provides an early, structured platform for residents to build skills in patient safety, teamwork, documentation, and continuous quality improvement while supporting institutional and national care goals.
