Risk of Venous Thromboembolism in Non-Hospitalized Patients With Covid-19

This study examined the risk of venous thromboembolism (VTE) in 398,530 adult outpatients with laboratory-confirmed COVID-19 across two large healthcare systems from January 2020 to February 2021. Using electronic health records and validated algorithms, researchers identified 292 new VTE events, including pulmonary embolism and deep vein thrombosis.<br /><br />Overall, the absolute VTE risk was low: 0.26 per 100 person-years. Risk was highest in the first 30 days after infection (0.55 per 100 person-years) and dropped substantially after 30 days (0.07 per 100 person-years). The authors note that this level of risk is only slightly above that seen in the general population.<br /><br />Several factors were associated with higher VTE risk after COVID-19 in outpatients, including older age, male sex, prior VTE, thrombophilia, inflammatory bowel disease, and higher body mass index. Prior VTE was the strongest predictor.<br /><br />The study concludes that although VTE can occur after outpatient COVID-19, the low overall event rate does not support routine pharmacologic VTE prophylaxis for all outpatients with COVID-19. Monitoring may be most important during the first month after infection and in patients with known risk factors.
