Resident Knowledge of Required Inpatient Social Drivers of Health Screening

This survey study assessed internal medicine and internal medicine-pediatrics residents at two Johns Hopkins residency programs to evaluate knowledge, attitudes, and use of the CMS-required inpatient social drivers of health (SDoH) screening. Since January 2024, hospitals have been required to screen all adult inpatients for five health-related social needs, but resident awareness and engagement with this process were unknown.<br /><br />An anonymous REDCap survey was distributed in fall 2024 to residents at Johns Hopkins Hospital and Johns Hopkins Bayview Medical Center. The survey examined knowledge of the screening requirement, ability to locate results in the electronic medical record, frequency of reviewing results, notification about positive screens, and whether screening affected care plans. Responses were analyzed using descriptive statistics and chi-square testing.<br /><br />A total of 78 residents responded, representing a 37.3% response rate. About one-third of respondents from each program participated, with similar representation across postgraduate year levels. Overall knowledge and use of the inpatient screening tool were low: only 25.2% knew that adult inpatients are required to be screened, 42.3% knew where to find the results, and only 20.5% had been notified of a positive screen in the prior six months. Most residents screened patients independently only when social “red flags” were present in the chart rather than routinely.<br /><br />Despite low knowledge and use, residents reported that screen results meaningfully influenced care: many said the information impacted care plans for at least half of their patients. There were no significant differences in screening knowledge by residency program, postgraduate year, or Health Equity track participation.<br /><br />The authors conclude that resident awareness and utilization of required inpatient SDoH screening are limited, suggesting missed opportunities to improve patient care. They recommend broader survey dissemination and development of interventions to improve resident knowledge and screening behaviors.
